vSphere 5 migration survey

I've already written my thoughts on the new vSphere 5 licensing scheme, and I think we can all use a bit more data to see what we're dealing with.

This survey was born out of the fact that the polls at Cloud-buddy were incomplete. It is based on rbtwindude's post on VMTN in the (seemingly never ending) thread on vSphere 5 licensing (now just over 47 pages and north of 35k views). The activity is so high it even prompted a user to ask for help on how to unsubscribe.

Without further ado, let's get on to the questions. Note that I've limited the choice of answer to one per question, though I realize that some may be running multiple clusters/farms as well as heterogenous environments (I have purposefully left it so that the survey can be filled more than once so that people can submit multiple times in these conditions). Please fill this out as best you can, and optionally fill out the remark section with your environment details (host details including make, model, installed RAM, etc).

Results based on 562 submissions (confidence level of 95% with 4.08% margin of error, statistics buffs please correct me if I'm wrong). The percentages haven't changed very much since the first two rounds of results, so having a larger sample size just reduced our margin of error.
